Add a rounds overview table (baseline HEAD / findings / fix batch /
test baseline per round), extend the overall conclusion to cover all
five rounds, and append a final-state section consolidating high-risk
disposition, maintained/deferred items, and the 490-test baseline.
Fix Batch H item count (13 -> 19) to match the fix table.
